19.2 Legal matters A number of Novartis subsidiaries are subject to various legal proceedings that arise from time to time in the ordinary course of business, including product liability, commercial, employment and wrongful discharge, securities, environmental and tax litigations and claims, government investigations, intellectual property matters, and other legal proceedings. As a result, claims could be made against them which, in whole or in part, might not be covered by insurance. While Novartis does not believe that any of the matters will have a material adverse effect on its financial position, litigation is inherently unpredictable and excessive verdicts do occur. As a consequence, Novartis may in the future incur judgments or enter into settlements of claims that could have a material adverse effect on its results of operations or cash flows in any particular period. From time to time, Novartis subsidiaries may be subject to government investigations arising out of the normal conduct of their business. Consistent with the Novartis Code of Conduct and policies regarding compliance with law, it is Novartis policy to cooperate with such investigations. In the interest of transparency, Novartis is providing information on the following matters: Product Liability matters HRT Litigation Novartis subsidiaries are defendants, along with various other pharmaceutical companies, in approximately 177 cases brought by approximately 1, 425 plaintiffs claiming to have been injured by hormone replacement therapy HRT ; products. Discovery is underway in these cases. SMON Subacute Myelo Optico Neuropathy ; In 1996 a subsidiary of Ciba-Geigy, one of the predecessor companies of Novartis, together with two other pharmaceutical companies, settled certain product liability issues related to sales of its product, for example, cordarone com.

Revisit your list of how nightlife activity affects patients you see and services you provide. Are there any issues you had not previously considered? If so, add these to your list. Brain and nervous system The brain is highly vulnerable to the effects of alcohol and illegal drugs. After one or two alcoholic drinks, individuals experience an increased sense of wellbeing, reduced reaction time and mild disinhibition. However, as blood alcohol levels increase, intoxication results in reduced social and physical control, contributing to an increased vulnerability to accidents, assaults and antisocial behaviour. Extreme consumption 500 + mg alcohol 100ml blood ; can result in coma and, occasionally, death Table 1 ; . In days after binges, some individuals experience secondary effects such as `blackouts' or amnesic episodes and, rarely, ischaemic stroke Puddey et al 1999 ; . Long-term excessive use of alcohol can lead to permanent brain damage Harper et al 2003 ; . Recreational drugs also alter users' moods and feelings. The acute effects of using recreational drugs, for example cannabis, can include paranoia, hallucinations and anxiety, while some drugs, for example, cocaine, are associated with violent outbursts Fals-Stewart et al 2003 ; . The safety of onion is reflected by its worldwide use as a vegetable WHO, 1999 ; . COMPARATIVE EFFICACY: Not available. LITERATURE REPORTS: The co-administration of onion with a high-fat snack had positive effects on alimentary hyperlipidemia. Ten healthy adult males consumed 100 grams of butter with four slices of bread and serum cholesterol, plasma fibrinogen, and fibrinolytic activity were assessed. The subjects randomly received onion juice or its ether extract or garlic juice or its ether extract. Since onion and garlic were not directly compared, only the results of onion administration will be presented here. Onion was ingested as a single dose and was freshly extracted from 50 grams of onion; the essential oil was prepared by ether extraction of the juice and encapsulated. Simultaneous consumption of butter and onion juice resulted in an increase of 9.8% in coagulation time, which from butter alone had decreased by 13.3% p less than 0.01 fibrinolytic activity increased by 15.7% after the initial decrease of 48.6% from fat-only administration p less than 0.005 ; . Plasma fibrinogen levels increased by 28.4% after butter administration and onion juice again reduced these levels to baseline. The rise in serum cholesterol produced by the fatty snack was completely prevented with onion juice co-administration p less than 0.05 ; . Results of administration of onion extract mimicked those of onion juice Bordia et al, 1975; Bordia et al, 1972 ; . Onion and chaunga effectively ameliorated the hyperlipidemic effects of a highfat diet. Eight healthy subjects consumed a high-fat diet for 7 days and then were co-administered 100 grams of onion or 100 grams chaunga daily for the next week. The high- fat diet resulted in a significant increase p less than 0.05 ; in serum cholesterol SC ; of 17.6%, in serum triglycerides STG ; of 15.8%, and in serum total lipids STL ; of 12%. Co-administration of onion for 1 week lead to a decrease in SC of 12.2% p less than 0.05 ; , in STG of 12% p less than 0.05 ; , and STL of 7.9% not significant ; Bakhsh & Khan, 1990 and endep, for instance, cordarone interaction.

Products provided they have the same price or lower than the domestically sourced alternative. All pharmacies in Sweden are part of a State owned monopoly the "Apoteket" ; , and all products are distributed by one of two wholesalers, regardless of whence the product is sourced. This means that individual pharmacists have no direct financial incentive to substitute pharmaceuticals for more inexpensive alternatives. To counteract this, the Apoteket instructed pharmacists to dispense a parallel sourced alternative where this was both cheaper and available, and from October 2002, substitution with the lowest-priced paralleldistributed alternative became law provided the doctor does not block the substitute on medical grounds and the patient does not decline to accept it. If the patient does refuse the substitute they will be required to pay the price between the cheaper alternative option and the more expensive alternative Despite the absence of any strong incentives to use parallel-sourced products prior to October 2002, growth in parallel-distribution was strong between 1997 and 2002. This is illustrated in Table 3.1 below. Table 3.1: Growth in the market for parallel-distributed products in Sweden 1997-2001 valued at AIP, for instance, glycosides.

Neurological Toxicity Peripheral neuropathy could occur in patients on long term high dosage generally over 400 mg day ; regime see `ADVERSE REACTIONS - Nervous system' ; . Intracellular inclusion bodies, similar to those seen in skin have been demonstrated in peripheral nerve fibres. Sensorimotor neuropathy, with a glove and stocking distribution, and myopathy have been reported in patients. Histologically, segmental demyelination of the nerve fibres has also been demonstrated. After discontinuation of the drug, the neurological complication is slowly and incompletely resolved. Use in Renal Disease Renal excretion of the drug is minimal. This suggests that modification of the dose of amiodarone in patients with renal failure is unnecessary. Hypotension Hypotension may occur when Xordarone X is given by the intravenous route. In some cases, hypotension may be refractory, resulting in fatal outcomes see `ADVERSE REACTIONS Coordarone X Intravenous ; . Carcinogenic Potential In a carcinogenicity study in rats, amiodarone caused a dose related increase in thyroid follicular tumours adenomas and or carcinomas ; in both sexes. Although mutagenicity findings were negative, an epigenic rather than genotoxic mechanism is proposed for this type of tumour induction. In the mouse, carcinomas were not observed but dose dependent thyroid follicular hyperplasia was seen. The relevance of these findings to man is unknown. Clinical experience has indicated that amiodarone can affect thyroid function. Use in Pregnancy Category C Because of the long half-life of amiodarone and its major metabolite, and the potential to cause abnormal thyroid function and bradycardia in the foetus, its use is probably best avoided in the three months before and throughout the duration of pregnancy. Where exposure of the foetus is unavoidable, thyroid function including TSH ; should be assessed promptly in the newborn infant. No teratogenic effects have been observed in animals. The drug does cross the placenta. In one study a 35 year old woman administered amiodarone in the last weeks of pregnancy, transplacental passage of amiodarone and desethylamiodarone was found to be 10% and 25% respectively. Changes in maternal thyroid function were similar to those seen in other patients receiving amiodarone therapy see `ADVERSE REACTIONS - Endocrine' ; but there was no evidence of clinical hyperthyroidism. The baby's TSH level on day 4 was normal and it had no goitre and was clinically euthyroid. However the authors caution the use of amiodarone in pregnancy or in those likely to conceive whilst on amiodarone therapy. The long half-life of the drug requires that the drug be stopped several months before conception. The possible adverse effects of amiodarone on the foetal thyroid are of concern since administration of iodine of which there are 75 mg in a 200 mg dose of amiodarone ; during pregnancy may cause foetal goitre, hypothyroidism and mental retardation. Another patient received 800 mg amiodarone for 1 week maintenance dose thereafter was 400 mg daily ; in her 34th week of pregnancy. Neonatal levels of amiodarone were 25% of the maternal level. Although the infant's liver and thyroid function tests were normal it was bradycardic during labour and for the first 48 hours after birth. Amiodarone is contraindicated in pregnancy. Use in Lactation As amiodarone and its desethyl metabolite are secreted in breast milk and its safety in the newborn has not been established, it should not be given to nursing mothers. If a situation demands that amiodarone be given to a nursing mother, alternative infant feeding should be instituted. Interactions with Other Medicines Cordarone X PI #63316v4 Page 5 and strattera.
By recipient countries--their governments and people--and they must reflect their priorities, " rather than those of donors. Participatory processes which engage civil society and the beneficiaries of aid are essential to establish locally owned priorities for development co-operation. These references reflect the centrality of local ownership in the new comprehensive development assistance model. The prominence of the phrase is not matched by depth of analysis, explanation or scrutiny in the policy statements, however, nor by reference to the literature on the subject. The policy references are more metaphorical than analytical.1 The literature directly addressing local ownership is, interestingly, modest in size; a much more extensive literature approaches the idea indirectly by analyzing related themes such as partnership, participation, and aid conditionality. The origins, conceptual assumptions, operational components and practical implications of the ownership agenda remain largely unmapped. This is especially so with respect to the role and relevance of CSOs, both Northern and Southern.

The formulation. Formulations with minimal or no semivolatile constituents will likely be unaffected. Those formulations containing significant semivolatile compositions are much more likely to be affected. These past studies demonstrated that the TSI 3306 3321 system can be used to obtain FPM4.7 m values similar to the ACI. However, it has recently been determined that the original cut-point supplied in the Model 3306 design was smaller than the labeled 4.7 m. As a result, the quantification of FPM4.7 m may have been inaccurate, most notably for MDI formulations with particle size distributions approaching the cut-point. The objective of this investigation was to quantify the differences between the 2 different cut-points used with the Model 3306 and to reevaluate the utility of inlet extensions when comparing FPF4.7 m data to those obtained from ACI testing. In addition to varied solution formulations, this study demonstrates the feasibility of using the TSI 3306 3321 system for characterizing suspension MDIs and imuran.

Annals of General Hospital Psychiatry 2003, 2 Suppl 1 ; : S91 Background: The provision of medical advice in psychiatric patients, using the advanced services of the internet. Material and Methods: This is a retrospective, population based study in 261 psychiatric patients from a random sample of 1500 patients that visited our medical site, seeking medical advice using the electronic mail, teleconference or IRC private channels. A team consisting of two general practitioners, two psychiatrists, a neurologist and an internist, was held responsible for the processing of the requests. The following medical data were considered as absolute minimum for the completion of the service: age of the patient, location, marital status, medical and family history, present and past complaints. In case of important data were missing, our team would contact the patient for further information and elavil.

It might be necessary to increase your dose of Lexiva if you are taking any of these drugs. Based on what we know about the drug interactions with Lexiva, it is likely that other anti-HIV drugs can interact with Lexiva. Anti-HIV protease inhibitors can interact with Lexiva. Norvir ritonavir ; , Kaletra lopinavir ritonavir ; , Reyataz atazanavir ; , Crixivan indinavir ; , and Viracept nelfinavir ; may all increase Lexiva levels in the bloodstream. If Lexiva is combined with either Norvir or Kaletra, the Lexiva dose should be reduced. At the same time, if Lexiva is combined with Kaletra, the Kaletra dose may need to be increased Lexiva may decrease the amount of lopinavir in the bloodstream ; . Invirase saquinavir ; may decrease the amount of Lexiva in the bloodstream. If Lexiva is combined with Invirase, low-dose Norvir may be necessary to maintain Lexiva levels in the bloodstream. Anti-HIV non-nucleoside reverse transcriptase inhibitors NNRTIs ; can also interact with Lexiva. Sustiva efavirenz ; and Viramune nevirapine ; may decrease the amount of Lexiva in the bloodstream. Because of the lack of easy access to computers by chemists in the early 1970s, Topliss[214] developed a nonmathematical, nonstatistical, and noncomputerized hence, manual ; guide to the use of the Hansch principles. This method is most useful when the synthesis of large numbers of compounds is difficult and when biological testing of compounds is readily available. It is an approach for the efficient optimization of the potency of a lead compound with the minimization of the number of compounds needed to be synthesized. The only prerequisite for the technique is that the lead compound must contain an unfused benzene ring. However, according to literature surveys at the time that this method was published, 40% of all reported compounds[215] contained an unfused benzene ring and 50% of drug-oriented patents[216] were concerned with substituted benzenes. This approach relies heavily on and values and to a much lesser degree Es values. The methodology will be outlined here; a more detailed discussion can be found in the Topliss papers. Consider that your lead compound is benzenesulfonamide 2.91, R H ; and its potency has been measured in whatever screen is being used. Because many systems are + dependent, that is, the potency increases with increasing values, then a good choice for your first analog would be one with a substituent having a + value. Because 4-Cl 0.71 and 4-Cl 0.23 remember, H H 0 ; , the 4-chloro analog 2.91, R Cl ; should be synthesized and tested. There are three possible outcomes of this effort, namely, the 4-chloro analog is more potent M ; , equipotent E ; , or less potent L ; than the parent compound. If it is more potent, then it can be attributed to a + effect, a + effect, or to both. To determine which is important, one term could be held more or less constant and the other varied. For example, the 4-phenylthio analog 4-PhS 2.32, 4-PhS ; would be a good test of the importance of lipophilicity, and the 4-trifluoromethyl analog 4-CF3 0.88, 4-CF3 ; would test the importance of electron withdrawal. If the 4-phenylthio analog is more potent than the 4-chloro analog, further increases in lipophilicity would be desirable. At this point a potency tree, termed a Topliss decision tree, could be constructed Figure 2.16 ; , and additional analogs could be made.
